Mycotic aneurysm is a rare disease. In spite of it¡¯s low incidence, mycotic aneurysm is a serious problem because of the high risk of rupture. Therefore, early diagnosis and management of mycotic aneurysms are imperative before the rupture. We experienced a case of symptomatic infrarenal aotic aneurysm in a 67-years old man. He was admitted via the emergency room with a complaints of fever and severe abdominal pain. Computed tomography (CT) demonstrated an 2.8¡¿4.0 cm sized aneurysm of the infrarenal abdominal aorta. Evaluation for endocarditis by the two-dementional echocardiography revealed intact valve morphology, and there was no evidence of infective endocarditis. After receiving 5 days of antibiotic treatment, the follow up CT revealed increasing size of aneurysm was (3.4¡¿4.2 cm). Immediately, resection of the aneurysm and axillobifemoral bypass was perfomed. He was discharged without any complication. We present a case of mycotic aneurysm of the infrarenal aorta and a brief discussion of the diagnosis and treatment from relevant literature.
