A Case of Mycotic Aneurysm of the Infrarenal Aorta

Mycotic aneurysm is a rare disease. In spite of it’s low incidence, mycotic aneurysm is a serious problem because of the high risk of rupture. Therefore, early diagnosis and management of mycotic aneurysms are imperative before the rupture. We experienced a case of symptomatic infrarenal aotic aneurysm in a 67-years old man. He was admitted via the emergency room with a complaints of fever and severe abdominal pain. Computed tomography (CT) demonstrated an 2.8×4.0 cm sized aneurysm of the infrarenal abdominal aorta. Evaluation for endocarditis by the two-dementional echocardiography revealed intact valve morphology, and there was no evidence of infective endocarditis. After receiving 5 days of antibiotic treatment, the follow up CT revealed increasing size of aneurysm was (3.4×4.2 cm). Immediately, resection of the aneurysm and axillobifemoral bypass was perfomed. He was discharged without any complication. We present a case of mycotic aneurysm of the infrarenal aorta and a brief discussion of the diagnosis and treatment from relevant literature.
